File of Life
What is the File of Life program?
The File of Life program is designed to potentially make the difference between life and death by providing vital information (medical concerns, medications, allergies, emergency contacts, etc.) to first responders in an emergency. File of Life programs have been implemented in almost 5,000 communities and 14 million homes in the U.S. The success of the File of Life format is its ease of use for citizens, and ability to be immediately recognized by local EMTs, police, and fire departments.
The Creedmoor Police Department has chosen to focus our File of Life program on senior and disabled citizens during the initial implementation. Citizens who register will receive a magnetic information pouch to place on their refrigerator, a personal/medical information card, and a window decal for their home. In addition to these valuable resources, our police department is taking this program one-step further with a strong focus on reaching out to our seniors and citizens with disabilities within the commUNITY.
Once the registration is collected, staff will add the information to our records so officers on normal patrols can check on registered citizens on occasion. This routine check-in will remind our seniors and disabled citizens they are important to us, our commUNITY, and further our efforts to strengthen the #teamcreedmoor family. Additionally, during these check-ins officers will be available to assist with simple tasks that aren’t as simple anymore; things like retrieving mail, rolling in/out trash carts, or changing a light bulb.
How do I register?
File of Life registration is simple and can be completed with a basic form that is returned to the Creedmoor Police Department. Forms are available at the top left of this page and are also available for pick-up at:
